– All Xrays, CT scans and other tests which may be helpful to the consultation
– A valid referral from your GP (Lasts 12 months) or Specialists (Lasts 3 months)
– Medicare, Veteran’s Affairs, Health fund information
– A list of questions about your condition
– Inform our staff if you have language difficulties
